Someone walks past you and does not like how you are dressed, then their negative emotion (and collective negative emotions of all those around you) will manifest as a "narrator" in your mind. Behind the "voice" is the feelings of those around you. Those trapped in the ego cannot separate themselves from verbal information in the mind. We would probably just sit there in the room, they would be clawing at the walls in a wild panic, like the apes that they are inside. Who wrote the words? I was put on Clozapine when I did not actually have psychosis but when I withdrew from clozapine I experienced very loud and frightening voices.
